■ 진료내역

- Hepatic arteriography Rt.(2015.2.13.)

[Finding] TACE #1

1. Celiac and SMA arteriography

2. Embolization of S8br.

Adriammycin/lipiodol(20mg/30cc) and gelfoam. Microcoils(2ea) were used for AP shunt.

[Conclusion] TACE #1 was done.

AP shunt가 tumor size에 비교할 때 매우 prominent해서 적절한 lipiodol uptake가 되지 않았습니다.

- Percutaneous RFA of Liver Tumors(Guided)(2015.2.14.)

[Finding] US guided RF ablation was performed for the lesion at S8 in the liver.

[Conclusion] US guided RFA was done.

 

■ 심의내용

○ 동 건 (남 /50세)은 MRI상 Liver segment 8에 2.8cm 크기 의 H C C 발 견 되 어 ' 15 . 2. 1 3. 간동맥 화학색전술(Transarterial chemoembolization, TACE) 시행 시 tumor feeder에 동문맥단락(Arterioportal shunt, AP shunt)이 동반되어 있었고, AP shunt로 약물이 빠져나가 불충분한 간동맥화학색전술(Incomplete TACE)로 판단하여 익일 추가로 경피적고주파열치료술(Radiofrequency Ablation, RFA)을 시행한 사례임.

○ 동문맥단락(AP Shunt)이 동반된 간세포암 환자에서 TACE 다음날 RFA 인정 여부에 대하여 논의한 결과,

○ 관련 학회의견 등을 참고할 때 TACE와 RFA의 병용 시술은 TACE 시술이 incomplete한 경우에 인정하되

① 3~5cm 크기의 간세포암 ② 2~3cm 크기의 간세포암(RFA 단독시술이 어려운 경우) ③ 2cm 이하의 소간세포암(초음파에서 보이지 않거나 안정적인 RFA 바늘 삽입이 어려워 RFA 단독시술이 불가능한 경우)에 인정함. 다만, 여러 개의 간세포암(일부 병변은 TACE로 치료하고 다른 병변은 RFA로 치료해야 하는 경우)은 사례별로 인정하는 것이 타당하다는 의견임.

○ 따라서, 동 건은 동문맥단락(AP Shunt)이 동반된 간세포암에서 TACE시 AP shunt로 약물이 빠져나가 incomplete TACE 소견 보이며 2-3cm 크기의 간세포암에 해당하므로 익일 시행한 RFA는 인정하기로 함.
